Here's what the centenarian decathlon is, the ten tasks in the planner, how training backwards works, and where to start.
What is the centenarian decathlon?
Attia asks his patients to list the physical tasks they want to still do in the last decade of their life. Then they train now for those tasks, with the decline of aging taken into account.
The list is personal. A grandparent picks different tasks than a hiker.

How training backwards works
Strength falls about 8 to 17% a decade, by Attia's numbers. So lifting 20 lb at 90 takes about 30 to 51 lb today at 40.
VO2 max falls about 10% a decade until 50, then about 15%. A hilly hike at 90 needs a VO2 max near 30, so with no training you'd need about 64 today.
Those are the averages with no training. Training bends the line, which is the point of the plan.
The four pillars
Attia trains four things: stability, strength, zone 2 and peak aerobic fitness. Each task in the planner leans on one.
Where to start
Find your zone 2 with the zone 2 heart rate calculator, time your grip with the dead hang test, and score the floor with the sit to stand test.
